Create Form Fields

This tutorial contains an overview of how to create custom form fields available on forms, advanced emails, custom logs, and online ticket/membership sales.


Adding a Field

Click the Add Field button. A popup window will appear allowing you to select the field type you want to add. After clicking on a field type, another popup will appear allowing you to specify the details of the field (see below for additional details). After setting up the field, click the Save button to add it to the form.


Reordering Fields

Hover over the field and click to drag the field up or down in the list of fields. Drop the field to place it.


Editing a Field

Hover over the field and click the Edit icon. The Edit Field popup window will appear allowing you to make changes. Click Save to save the changes.


Deleting a Field

Hover over the field and click the Delete icon. A popup will appear to confirm you want to delete this field.

Note: Deleting a field will also remove all submission data associated with the field. If you do not want to lose this information, you may instead choose to edit the field and turn off the Display toggle.


Field Types

  • Personal Fields

    • Name: first and last name fields

    • Address: street, city, state, and zip fields

    • Email: text field for email address

    • Phone Number: text field for phone number

    • Date of Birth: date field for birthdate

    • Gender: drop-down for gender

  • Data Fields

    • Date: date field

    • Time: time field

    • Multiple Choice: drop-down allowing one selection

    • Number: text field requiring a numeric value

    • Short Text: one line text field

    • Long Text: multi-line text field

    • Checkbox: checkbox that can be required

    • Toggle: toggle switch

    • Waiver: terms that can be accepted

    • USA Hockey Number: only applies to hockey leagues

    • Sub List: add player to sub list. Only applies to leagues.

  • Display Fields

    • Paragraph: text formatted through a text editor

    • Images: one or more images to include in a gallery

    • File: a PDF file to display. Often times a file is paired with a checkbox to acknowledge review of the document.

    • Line Break: a line to create separation

    • Spacer: add blank space between fields


Field Options

The options available for each field will vary, but below is additional information about each available option.

  • Label: field label

  • Contents: a text editor with the contents to display

  • File: select a PDF file to add to the form

  • Images: add one or more images to the form. Images can be added one at a time.

  • Gallery Settings:

    • Image Width: maximum width of image in pixels

    • Gallery Columns: number of columns in the gallery

    • Image Gap: space between images

    • Image Ratio: landscape, portrait, or square

  • Border:

    • Border Style: select a line style. Solid is most common.

    • Border Width: border width in pixels

    • Border Radius: add radius in pixels to create a rounded effect on images

    • Border Color: color of the border

  • Required: enable to require this field to be completed on the form

  • Display: enable to display this field on the form
