Incident Reports

The incident reports page allows organizations to document incidents that occur at their facilities and in their leagues.


Page Privileges

Organization Mangers, Assistants, and Official Coordinators have access to this page and can perform all page functions.


Page Filters

This page has one filter: Organization.


Incident Fields

  • Case Number: The case number will be automatically generated when you add a new incident record.

  • Date & Time: Incident date and time. The date/time of the game should be used if the incident occurs during a league game.

  • League: Incident league (if applicable). This field will be enabled once a date/time is entered.

  • Game: The game in which the incident occurred (if applicable).

  • Period & Time: The period and time of the period in which the incident occurred (if applicable).

  • Publish (leagues): Published incidents are viewable in an incidents table on the league website.

  • Participants Involved: Add all participants you wanted named as being involved in the incident.

  • Participants to Notify: These participants will not be named anywhere in the incident report, but will receive an email when you send out the report.

  • Incident Reviewers: These participants are named in the report as investigating the incident.

  • Incident Description: A description of the incident that occurred.

  • Incident Resolution: The resolution of the incident.

  • Media Links: This section allows you to include a link to a photo, video, or other document related to the incident and will be included in the report. If you publish an incidents on your website, the first link in the list will display in the Links column on your website.


Viewing and Printing an Incident Report

To view and print an incident report, click the View Incident PDF icon from the Actions column of the table. A new tab will open in your browser with a PDF of the invoice. You can download or print it from here.


Note: The incident report must be published to view and print it.


Sending an Incident Report

To send an incident report, click the Send Incident icon from the Actions column of the table. Enter the date/time you want it to send in the popup window and click the Send Incident Report button. The scheduled emails will be added to the Email Queue to send at the specified date/time.

Delete Incident

To delete an incident report, check the box next to the record in the table and click the Delete icon below the table. A popup window will appear to confirm you want to delete it, and if so, click Delete. Once deleted, the incident records cannot be recovered.
